Forme rare d’adénome hypophysaire bi-sécrétant à ACTH et prolactine : à propos d’un cas.
Résumé
Les adénomes hypophysaires plurisécrétants sont
relativement rares. L’association la plus décrite est celle
couplant une hypersécrétion de GH et de prolactine. Nous
rapportons le cas d’un patient qui présentait un adénome
hypophysaire bi-sécrétant de l’ACTH et de la Prolactine.
Il s’agissait d’un patient de 39 ans admis pour syndrome
tumoral hypophysaire, syndrome de cushing et
hypogonadisme. Le bilan hormonal retrouvait un
syndrome de cushing ACTH dépendant et une
hyperprolactinémie. Les autres axes hypothalamohypophysaires étaient sans anomalie sauf l’axe somatotrope qui n’avait pas été exploré. L’hypothèse d’un adénome hypophysaire était évoquée. Le scanner cérébral confirmait le diagnostic en montrant un macroadénome à développement supra-sellaire avec compression du chiasma optique. Après une chirurgie par voie trans-sphénoïdale, il y’avait une régression totale du syndrome tumoral hypophysaire et du syndrome de cushing. Cependant l’hyperprolactinémie persiste toujours 4 ans après la chirurgie et malgré un traitement médical par la bromocriptine (10mg/J).
