Amylose rénale héréditaire du lysozyme associée à une mutation W64R (TRY 64 ARG). A propos d’une observation

Résumé


L’amylose du Lysozyme (ALys) est une amylose héréditaire rare de
transmission autosomique dominante.
Nous rapportons un cas chez une patiente de 50 ans d’origine
piémontaise (Italie) présentant comme antécédents un mélanome cutané
superficiel et un polype endocervical. La symptomatologie clinique
avait débuté à l’âge de 35 ans (1998), par des troubles digestifs à type
de douleurs abdominales diffuses et intermittentes associées à des
épisodes d’épigastralgies, de diarrhées et de constipations. Au cours de
l’exploration endoscopique, des biopsies systématiques ont été
effectuées. L’étude histologique des fragments biopsiques digestifs et
salivaires mettaient en évidence des dépôts amyloïdes congophiles, par
ailleurs permanganates résistants, faisant évoquer une amylose AL. La
malade a été suivie comme telle depuis plus de 10 ans avec une
évolution inhabituelle. La découverte fortuite d’une thrombose de la
veine rénale gauche au scanner thoraco-abdominal avait conduit à une
néphrectomie totale élargie. Les dépôts amyloïdes observés à l’examen
anatomopathologique siégeaient dans l’interstitium et dans la paroi de
certains vaisseaux sanguins de la corticale rénale sans atteinte des
glomérules, faisant suspecter une amylose rénale héréditaire. L’analyse
protéomique associant la microdissection par capture laser des dépôts
amyloïdes à la chromatographie liquide haute performance couplée à
une spectrométrie de masse mettait en évidence une amylose au
lysozyme (ALys). Le séquençage du gène du lysozyme montrait une
mutation Try64Arg située au niveau de l’exon 2 du gène. L’évolution
de la maladie était stable avec un recul de plus de 3 ans.
Conclusion
L’amylose ALys est une amylose héréditaire rare caractérisée par un
polymorphisme clinique et un diagnostic difficile. Un suivi régulier
permet de stabiliser les lésions.
