Les marqueurs non invasifs dans l’évaluation de la fibrose hépatique chez des sénégalais porteurs chroniques du virus de l’hépatite B : à propos de 404 cas.

Contexte et objectifs
L’évaluation de la fibrose hépatique est une étape capitale dans la prise en
charge de l’infection chronique au VHB. Notre travail avait pour objectif
de comparer différents marqueurs non invasifs de fibrose dans une
population sénégalaise de porteurs chroniques du VHB et d’évaluer leur
performance diagnostique.
Patients et méthodes
Il s’agissait d’une étude prospective transversale impliquant 404 patients
porteurs chroniques du VHB. Etaient exclus de l’étude les patients coinfectés
au VIH, ou au VHC, et tout patient ayant eu un quelconque
traitement antiviral. Les scores APRI, FIB4 et GPR étaient calculés et le
Fibroscan effectué chez tous les patients.
Résultats
L’âge moyen était de 36 ans (±11,11ans) avec une prédominance
masculine à 60,9%. La valeur moyenne du Fibroscan était de 7,59 kPa
(±5,95) avec 59,9% des patients classés à F0-F1, 23% à F2-F3, 10,9% à
F4 et 6,2% de cirrhose. Nous avons retrouvé une corrélation positive entre
le Fibroscan et les scores biologiques de fibrose avec des coefficients de
0,145 pour APRI, 0,171 pour FIB4 et 0,170 pour GPR (p-value ˂0,05).
Les scores biologiques de fibrose avaient des performances moyennes
pour détecter une fibrose significative avec des AUROC respectives de
0.675, 0.650 et 0.593 pour GPR, APRI et FIB4.
Conclusion
Les scores GPR, APRI, FIB4 comparés au Fibroscan ont des
performances significatives pour l’évaluation de la fibrose hépatique au
cours de l’infection chronique par le VHB dans la population sénégalaise.
Des études similaires doivent être réalisées en Afrique sub-saharienne ;
afin de mieux évaluer leurs performances diagnostiques.
