Sclérodermie systémique : aspects démographique, clinique, sérologique et thérapeutique dans le service de Médecine Interne de l’Hôpital Saint-Louis de paris : à propos de 75 cas.
Résumé
cliniques, immunologiques et thérapeutiques de la
sclérodermie systémique (SSc) à l’Hôpital St Louis de
Paris, service de référence dans la prise en charge de la
SSc.
Matériels et Méthodes : Etude descriptive et
observationnelle des patients atteints de la sclérodermie
systémique recrutés dans la période 2013 à 2015 selon
les critères de l’ACR et de l’EULAR pour la
sclérodermie systémique. Les données ont été
recueillies sur une fiche comportant les variables
sociodémographiques, les signes cliniques,
radiologiques, biologiques, immunologiques et le
traitement.
Résultats : Il y’avait 16 hommes (21.33%) et 59
femmes (78.66%) avec un âge moyen de 49.27 ans. Les
patients avaient une sclérodermie systémique diffuse
dans 83% des cas et 12% une sclérodermie systémique
limitée. Les manifestations cliniques les plus
rencontrées sont le phénomène de Raynaud (80%),
l’insuffisance cardiaque (52%), les atteintes ostéoarticulaires
(38.7%), les ulcérations digitales (37,3%), la
pseudo-occlusion intestinale aigüe (8%) et le reflux
gastro-œsophagien (4.1%). Les anomalies radioimmunologiques
comprenaient la fibrose pulmonaire
(50%), la pneumopathie interstitielle (45.7%), les autoanticorps
anti-nucléaires (94.1%) et les anticorps antitopo-isomérase1
(40.8%). 13% des patients avaient une
protéinurie des 24 heures au-dessus de la normale. Les
corticostéroïdes (49.7%), le Cyclophosphamide (9.3%),
le mycophénolate mofétil (6.66%), le méthotrexate
(5.32%) et une greffe de cellules souches
hématopoïétiques (1.33%) constituaient les traitements
de fond des patients.
Conclusion : la sclérodermie systémique est une
maladie auto-immune pouvant atteindre plusieurs
viscères à la fois et son évolution est peu freinée par les
thérapeutiques actuelles, elle reste encore redoutable.
